This adds parameter highlighting for reference parameters and defaulted
parameters. For example:
```cpp
auto strip_prefix_only(std::string& s,
Hidden_Homonym skip_hidden_homonym = {}) const
-> Affixing_Result<Prefix>;
```
Previously both parameters were only highlighted as variables.
